If we miss you, please leave a detailed message that includes your name, phone number, and the best time to reach you .We make every effort to reach new patients as soon as possible during clinic hours.
In order to better serve you, here’s a quick checklist of items to prepare before your first visit:
- Download the New Patient Forms, complete and sign them, and bring them to your first appointment.
- Confirm coverage for your appointment with your insurance company.
- Bring your Government Issue photo ID card ie State Issued Drivers License etc
- Bring your health insurance card and method of payment; cash, check or credit card.
- Bring any records such as current labs, X-rays, MRI reports and any other records you feel are important to your medical care.
- Bring a list of your current doctors who may have records that would be helpful for us on your care.
Come prepared to fill out new patient information paperwork. If you decided to fill out the paperwork before hand, remember to bring the originals with you to your appointment. The credit card form MUST be filled out. For OUT-OF-NETWORK patients, payment is expected at time of service. New patient visits with Dr. Zeiger is $400 for the consultation only, not including testing, treatment or supplements.
Please be prepared to spend at least an hour and a half to two hours at your first appointment.
